4" will be fine and 6" pads where your lift will go. 4' grid of 3/8 rod, 4000 psi is all we do, it is not going anyplace. Myself I wouldn't waste any $$ on fiber. Interior concrete doesn't see 90% of what mother nature does to exterior concrete. In this part of the country our charge for this...
Seems high to myself also. Where are you located, just curious. I just ran the numbers and our price would be around $2500-$3000 w/ tearout. Somebody is making good money doing concrete for $6k. Lol
Around here we have a curb cutting service. They cut the curb horizontaly and we remove the first 10' of driveway. We pour the new section of drive from point A to B and when done you only have a 2'' or less rain gutter bump. I would think they would have a horizontal curb cutting service...

A Concrete diamond blade will do the job fast and easy. We cut concrete and anything steel with the same blade. 14" blades we use. The blades cut steel faster run concrete lol
I have had this problem since I put up my lights..drove me nuts. ran my antenna wire all the way outside ....nothing special just the wire hanging out the steel siding/roof seam and comes in crystal clear now
