That's a special wrench for bolts where the head's been sheared off.

Manufactured by T.Williams in Birmingham, U.K. Once quite a major manufacturer of wrenches.
Not to be confused with J.H.Williams, the U.S. manufacturer.
Some of the wrenches are really very nice, and feature superb plating, but Williams made a lot of cheaper examples too.
That would have been one of the better ones, if finished.

Think thats for knocking the stripped head off a bolt when the correctly finished wrench didnt work? Looks like a factory defect in final steps
