I think most are talking about Scaffold Ratchets like this Wright, which have been around for decades, some other brands I've seen have a pry par as well:

As far as these things not doing both functions well...some of you have obviously never set up a tall scaffold. When you are banging and wrenching 20 feet in the air (scaffold of any age needs some persuasion to go together and the fasteners are always rusty), it's good to have one hand free to hand on to something. You are either going to hammer with a ratchet made for it like these or you are going to hammer with one not made to hammer with.
