Floor jack identification needed

Hi,
My first post on what looks to be a cool forum I discovered yesterday. Can anyone help identify the make/model and capacity of this jack?

My neighbor gave it to me when he sold out and went south for retirement. It was in bad shape and hadn't been used for years. I've freed up things and am putting it back together, but might need to replace some soft pieces, now or someday. Also, the other geared drive (not pictured) has a tooth broken off and I'd like to find a new one.

Looks like it used to have a label but it is unreadable.

Need the 1/4" ball bearings too. I'll look locally for them, but we don't have a local jack repair shop anywhere near.
