My guess then is the handle reaches over above the screw, and the "blade" drops 90° down into the slot.

Yes, Cornwell made Phillips offset L drivers too.

They work just fine. They're way more efficient than modern offset screwdrivers.

I just grabbed a similar pic, no idea what brand I actually have.

But I also did not exist in the 1940's. Thankfully I never had to use what looks like frustrating tool on slotted screws (which I despise on their own)
