Flexsteel (or some similar company) desk lamp from the 80's, they originallyhad round buttons on top, this was a newer design, the older ones were solid cantilevered arms and a moveable ball joint thingy head mount. You held one button down while the tubes warmed up (you'd see the ends glowing) and when you released it the tube lit. The other button was to turn it off. If you blinked it just right you could get one tube not to fire.
We had these in our dispatch center back in the early 90's.. of course, fluorescent. We'd tape paper over the opening to cut down the light, I ended up replacing one of the tubes with a blacklight tube, used to piss people off when their docs started to glow!
