is the vapor barrier laid flat or turned up inside the forms?
what is the spacing for 1/2" rebar plastic chairs for a 30x30 with rebar every 24 inches?

There is really no need to turn the vapor barrier up on the inside of the forms. The important thing is to tape all seams and penetrations.

I'm unclear on what you mean by "1/2" rebar plastic chairs". if your referring to 1/2' or #4 rebar, it should be supported every 4' or so. The chairs should be tall enough to hold the rebar in the center of the slab.
