Oh, agreed 100 %. It's so wide it would have slipped way before this. I have to wonder if it wasn't then employed on a larger size with same force. The wrench is discolored, perhaps from fire. I'm wondering if that's the case and it lost it's tempering.A pipe on the other end did that.

BINGO! This makes the most sense as the open end would have slipped WAY before opening this wide. Not a practice I often employ so didn't even cross my mind.More likely used as a box end with another wrench on the open end for leverage.
