If you're talking about a surface mounted box, with a surface 1/2" 'raised' cover, then some inspectors look at differently. Some will pass it if the cover is the same brand as the box (ie: Raco, Steel City).
Some look at as "the cover might separate from the box if the mounting screws loosen" (a definite possibility) and won't allow it.
To be on the safe side, run the ground wire from the receptacle to the box. Grounding pigtails are cheap insurance.

That is new to me, I was under the impression it needed it if it had the device mounted in the cover.
Well I guess I will probably add it, helps to hold the receptacle while I am wiring those boxes mounted on the ceiling!
I was going to avoid having to buy a roll of solid green THHN (I don't like wrapping stranded!) but I guess I can buy these as well since it comes with a screw already.

If you have Menards, they have the stranded 12awg jumpers with 10-32 screw and a fork terminal on the other end. I believe Ideal also makes a stranded version of same.

I twist them around screws all the time. reverse twist it and its pretty secure. The outlets are designed a bit different but twist hundreds and even thousands of them and never a problem. I got a box of green screws and a box of forks too, make them up with fork on one end all the time, usually from scrap wire, takes a minute. In some places even crimp an eye on it.
Code says the cover is good enough, if the device is attached to the cover with suitable fasteners (which include the serrated nuts included with good quality covers, but also things like rivets), and the device is listed for such use (which means it's got a copper jumper around one of the screw holes, in most cases.). 250.146 is very clear on how to meet the requirements.
Wire the device on the ground, with pig tails. Then you just need to attach your wire nuts, and screw the cover in place.
