Are you sure you're using the correct terminology? Couplers are used to connect pieces of conduit together and you wouldn't bend an offset with them. Connectors on the other hand connect conduit to boxes and you would definitely bend an offset with them unless you use stand-off straps (mineralacs, minnies).
As far as min distance from 90 to a box there isn't any official rule, but there effectively are limits based on the take-up of the 90, which is different for each conduit size. You can't put a box any closer than 5" to a 90 on 1/2", 6" for 3/4", etc.
