It is for a pilot hole, so you can then inject yourself a Tetanus injection in an exact location.

It has been hanging in my garage for 40 or so years just as a curiosity item.

Yep! Maybe not from that though.

Here's another stumper, hopefully more difficult. I was told it was a home made version that works well but I've never used it.

back in the past, you were lucky if kitchen tools had a nice healthy sanitary plating of lead on them to keep out germs.

or was that cadmium? aka silver-ish plating?

For scale here's a quarter. There are 4 holes when clamped. One on each end, one on the bottom and a larger one on the top.
