Check the Johnbridge forum, they might know someone in the chicago area, and if they are on that site they probably know their stuff.

What suburb are you in? I know a really good installer, but he's out in St Charles

Sloppy grouting for sure. Acid wash the dried grout or sometimes just some hard scrubbing with a towel and re-seal the floor.
In defense of the tile setter getting correct pitch in such a small area with large tile is no fun.
