i found the problem. A short time ago while cutting a big, unwieldy part, at the final 1/8" the steel twisted out of the vice. The steel then was pulled in contact with the guide rollers and forced the blade off the wheels. Yup, scary to say the least. A close inspection revealed a tiny crease...
