My local store did not charge to load, guy came out with the forklift and helped load it into the back of the truck , no problem.

I would go with SKF brand. Good quality for the money. We use them on most equipment at work.

It looks to be missing the swivel base. The stud/bolt in the center mounted the vice to the base and the hole on the side was for the swivel lock. Google image search and you will see what I mean. As far as mounting, not many choices as it sits. Use the stud/bolt to mount and add another bolt...
