Here's my guess. A wider picture would help but it appears to be an older model S4 because the lifting arm is straight, as opposed to the image posted by 930 dreamer. You can see the lifting arm in that pic is curved, placing that jack circa 1950's.

Sorry I have been off the air for awhile. Unfortunately the images are no longer available, and I don't know if you still have the lifting arm. Without the images I can't readily determine if your jack is the right style. I would appreciate a re-post or PM me and we can talk. Again...
