There is no locking or unlocking sequence or rotation required to activate anything, the ratcheting end is 12 points, 6 of which let you go past a bolt head or nut should you choose, 6 of them keep it on top of the nut or bolt like a shelf, similar to the Joker.

This is all I could think of, though can't think of a scenario that exists or that the 1st nut should stay on there and not also be moved.
