12 Combo or 6 Double Open + 6 Double Box

Does it make any difference? 12 pieces of combination wrench, or, a set of 6 double open ends plus a set of 6 double box ends?

If you work with bolts and nuts then yes. You may need two of one size, one for the bolt and one for the nut. If the two sizes needed are on the same handle it won't work. Same also applies to double nutted fasteners.
