2000 Ford E150 What the heck is this piece?

2000 Ford E150 handicap conversion van. Automatic with V6.

I was driving along, and this fell out on the floor driver's side step. Heavy duty piece well-greased. Everything seems to be working OK. Before I start tearing things out does anyone know what this is for?
