OK so we have a 5.5mm thread, now how about the 4.5mm? That one seems even more obscure? What applications require that size?

No idea, nut sizes go as follows (mm):
M1.6 => 3.2
M2 => 4
M2.5 => 5
M3 => 5.5
M3.5 => 6
M4 => 7
M5 => 8
Mind that M3.5 is not typical (probably obsolete, or "repair" thread) and threads below M3 are only in small, precision equipment (and M3 mostly too).
PS: found also non-ISO metric (DIN 934):
M2.3 => 4.5 - your size
M2.6 => 5
M1.6 => 3.2
M2 => 4
M2.5 => 5
M3 => 5.5

M4 => 7
M5 => 8
