Air Hammer 114GQC - damaging chisel - Stuck

I have an IR 114GQC air hammer. It seems to be damaging the retaining lip of the chisels. I'm sure many have experienced something similar. Sometimes, I have remove the chuck to hammer the chisel back out.

I bought a new 114, but before I put the chuck on and start using it, is there something I have done wrong? Did I put the chuck on too far? Wrong chisels? To much light hammering and of course some dry firing because....well because.

Thoughts?

The quick change chucks can damage the lip, mainly due to allowing the bit to bounce against the retainer balls. Just use a belt sander or grinder to remove the mushrooming, it'll be fine. And remove any from the end of the bit that goes into the tool, I've seen them get stuck in the barrel due to that.
