Another "What is this used for" question=Ratchet

Not sure where I got this but it's been in my tool junk drawer for 30+ years. I moved from Michigan to SW Oregon in 2003 so it's most likely from my grandfather's farm.
No markings on it except for the 2 dots. It only drives counterclockwise, ratchets clockwise. Odd parts the square drive is 5/8" and the thread below the cone is 5/8" by 8 TPI. The handle is solid 1" steel.
Thinking it might be for removing 5/8 square plugs, but why the adjustable cone?

I also thought about it being used for pipe threading or tapping fittings but it only turns counterclockwise and has no means to switch directions. The head is solid. it only loosens unless it's for lefthand threads.
