The screw-in plunger indicates it was used with Alemite "pin-type" grease fittings. They had a ball check and a cross pin that the coupler engaged to hold a seal. They became obsolete with the invention of Zerk (ball-type) fittings.

If there is no thread on the end of the tube, it may have been used to refill grease cups.

I'd polish it up, put it in the bathroom and tell people it is an old medical device.
