I finally managed to identify this little guy after finding it in a Par-X tool box set I bought. I assume it's 30-40 years older than the box. Since I didn't see this particular adapter mentioned on the site, I thought I would show a picture and a link to some auction site that sold one in much better condition and took better pictures.
