It looks like the patent refers to the deep offset, and that the design allowed using both the top and bottom of the opening to access the fastener. The drawing shows how the offset allows reaching a fastener that is on the bottom of a flange, with the offset allowing access when a wrench with less offset is prevented from reaching that fastener due to obstructions near the flange.
The offset would be useful in reaching a recessed spark plug, hence the use by BG.
OTG, I've always liked the look of the mica insulators. BG had a sense of style that included the rest of the plug, especially the earlier ones. I could see them used to make a charm bracelet!
