We'll never know who failed to fully seat the meter jumper plate, and it doesn't really matter.
It might have been the electrician who worked on the customer side wiring, or it could have been the utility company guy looking at the socket when he came to install the meter.
Even though the homeowner could in theory have opened the (unsealed) meter ring and re-seated the plate themselves with a firm push, this was clearly a situation best left for a licensed electrician given all the safety and liability aspects involved (even more so given the strict "no DIY electrical work" regulations here). Having the electrician come also helped resolve the mystery of the unfinished installation.
