I have seen pics of an attachment for a bench vise that allows a user to bend rod, tubing, etc in small sizes. I'm wondering if anyone has one or knows where to get one.

That belongs to Don the site owner of Practical Machinist. There is a vise thread on the forum you could ask him about it.

Here is one that I have but it is from the 50's. Works pretty good. You might try searching the auction sites for Metl Former.

Here is a PDF of the blueprints / patent for the one you are showing if you are a machinist http://www.google.com/patents/about?id=1JVgAAAAEBAJ&ie=ISO-8859-1

I did find this from Lisle and Made in the US http://usatoolsinc.com/tubingbender.aspx

Here is one that has tube bernding and attachments that some can be mounted in a vise (slow loading / Large PDF) Might not have the OP's style but does have some ideas on what one may need for bending tubing and flaring. http://www.airlinehyd.com/apollo/parkerfc/4300jun2003/S-Equipment.pdf

Here is a site with a lot of pages on small tubing benders. You may find it in there http://www.thefind.com/hardware/info-tube-bender#page=1

Made in :canada2:.

[EDIT] I realize now that this isn't the greatest application for pipes and rods, as you asked.
