Which box?

Wiring will be from backside, box surface mounted.

I don't care about seeing the box if that's what it takes.

I tried search, didn't see anything.

Would one of those shallow "pan" boxes work?

Would like to screw into bottom of trusses.

What kind of fan? Industrial type that uses a j-hook or a house fan that mounts to a box?

Probably the home depot industrial one, 60" I think.

I'll look for the link.

Well most of the metal industrial fans take a j-hook or lag that you run into the rafter through the back of the box. In that case the fan isn't held up by the box so you can just us a 4"x1 1/2" metal octagon box on the surface.

If the fan attaches to the box then you need a fan rated box that is more heavy duty than a regular box.
