I don't have a number for you, but they were probably made by Waterloo Industries. Call them up and they can probably help you out. If you can find the original part number for the box that'll help.
I went through this a couple years ago with a late 70's / early 80's Cman box. I called their customer support and they were very helpful and helped me figure out which slides I needed and shipped them out. I didn't have a part number so I took some pictures and emailed them and they helped figure it out. Extremely good customer service!
