That is always how it has been for me around here. Everything is replaced with new except for the ratchets. I have gotten a few ragged looking ratchets before as exchange, but the guts inside are new so no worries!

When I take tools for replacement, I pull the replacements myself off of the shelf and take them with the old tools to the register. I have NEVER received rebuilt ratchets using this method, as a matter of fact the last long 3/8 ratchet I took in they did not have in stock and I upgraded to the polished version that was like $10 more with no questions asked.
From some of the posts that I read here I honestly believe that your results at sears are directly related to the way in which you handle the situation.

I keep my tools in good shape
I will not allow them to give me **** beat up stuff for my nice stuff.
in the past I have had them give me new and when they have refused to do that I have told them that I will wait while they go rebuild mine or they can just give me the kit (which they wont)
generally they wont argue and they will go rebuild mine

Up until 5 years ago my sears would rebuild the ratchet. Now they give you a factory overhauled ratchet with no other options. ***** a little if you ask me.

I had a ratched replaced. They pulled one off the shelf.

The warranty means you get a new one, not a half-used one when you take it in.

the last 3/8 ratchet i turned in they gave me an older more beat up one that had been rebuilt. its just one of my junkyard ratchets so im not too concerned about the looks i just want it to work. if i turned in a mint cond one i would like a mint looking one back though
