Ground would go to frame, so I would use the small Phillips. Remember, 240 is both hot, and "usually" white means neutral. Make sure you don't accidentally feed it 120V or it won't work.

Its a 220v only motor (single phase). You'll want to make a cover plate for the rectangular opening were looking at. Yes, connect ground to one of the small Phillip screws for the cover plate or to one of the larger frame bolts near the box. The power cord would normally be fed out the round hole (seen to the right) on the junction box but you could also include a hole in the cover plate but shouldn't need to that.

Use a rubber grommet sized for that hole for the power cord and figure out a strain relief lock inside box for power cord to prevent it from ripping out if pulled on.

Also may want to pay attention to spindle rotation

Cruzan and macgee, thank you both! First thing is to see if it runs, so to the store I go, to get the stuff to make the cord.
