Duke Energy Load Sheet

I'm building a small shop (600sqft) and have to have a new meter because my house panel is too full.
Duke Energy needs me to fill out a load sheet and I can't get in touch with my electrician, has finished his part and passed inspection, and now I can't get in touch with him. Can anyone help me fill in the parts that are in the attached pic?

It is a hobby shop with normal woodworking tools.

Any help would be great. Thanks!

your picture is too small to read.
just because your panel is full doesn't mean you need a new meter necessarily.

I have added the actual PDF. Page 2 is what I need help with. Duke Energy has told me this is the way I have to go, so I just need help filling out the sheet.

I'm not sure what additional info you're asking for help on, but you will almost certainly be running your dust collector in addition to one of those other tools at the same time...should probably clarify/specify that.

They said thats the way to go because your breaker panel is full?? Makes absolutely no sense...

Have you done a load calc on the existing service first? I bet its more than adequate to add a small shop

The form you've got there makes me think they'll be billing you at commercial rates. Do you know what they are vs. your residential rates?
