embedded plate concrete?

I have a granite memorial plate 3/8" thickness

I would like to set it in piece of concrete which I will build form for, so the whole thing will have a lot more weight.

I don't know how to do it, do you just sink it in, but when during curing to do that?

I have no real world clue, but, I'd think it should have some kind of "pins" on the back of it to help anchor it in. Instead of just sinking a "thin" plate down into concrete.

Although I have put pennies in many outdoor pours that are still there.

I may use thinset to install it like a tile on top, if it is hard.

I also saw a version of the plate is actually just an engraved porcelain tile.

Build the form from melanine, put the memorial in the form face down, silicone around the edge to stop concrete from getting onto face of plate, pour concrete and vibrate to eliminate bubbles.

Watch you tube videos on casting concrete countertop.
