I'm sure it's an install error. I just want to know what I can do to prevent this next time because I still have a small section I need to do this weekend. I started putting on the base coat at 8:30pm, and finished at 11pm. Next morning, there's all these little spots where you can see the bare concrete (see photo0,2 before clear coat). It's alot less noticable after the top clear coat was put on. Overall, I'm very satified with the results.

How old is the slab?
What did you do for prep?

can't see jack. is it the darker spots that look like they happen to be shape of flake that somehow moved

Yes, it's the darker gray spots. Those are not flakes, but the concrete.

The slab is 35 years old. Here's what I did for prep:
June 24 - diamond grind for 6 hrs using diamabrush concrete prep tool with floor buffer, and wash floor afterwards using garden hose
July 7 - fill holes and cracks with HD-105
July 8 - use hand held grinder with HF diamond turbo cup to level the repairs;
July 8, 10, 11 - spent hours hand grinding entire floor to get a rougher surface just to be safe (the surface left by the diamabrush didn't feel rough enough, about 150 grit)
July 11 - use Simple Green to scrub floor, then spent 2hrs washing it using garden hose
July 13 - did moisture test with masking tape, saran wrap
July 14 - moisture test passed; vacuum again, waited until 8:30pm to start when temp was 75 deg.

looks like either moisture or a chemical residue, How high was the humidity on the day you did it?

I downloaded it and blew it up till it crossed my eyes. Seems to be some kind of contaminate causing fisheyes. The simple green could have caused it. Did the simple green dry before washing it off? GM hit an idea also. Heavy moist air will "part the seas"
It really looks good by the way. We are our own worse critics when it comes to this stuff.

I can see it on my home monitor but could not on my phone.

Did you prime before you put the base coat down? Looks like the basecoat is not wanting to stick to parts of the floor. Perhaps an oil or chemical still on the floor?

I wonder how your clear top coat will adhere to bare concrete in the long term...

I didn't measure the humidity, but it's not humid here in Silicon Valley on the west coast.
I washed off the Simple Green immediately, and didn't let it dry first.
It's just here and there, not consistent throughout.

I didn't prime. My premium kit did not include a primer, and there was no mention of using a primer in the instructions.

"Fisheye" is caused by many different reasons, silicone, oil, grease, animal fats, etc...
One of the worst offenders is Armorall being used in the garage.
Sometimes it cannot be avoided. An experienced installer will try to roll over it aggressively but sometimes it happens once your spikes are off and the lights are off.

From the pics you shared the floor looks really nice and unless it bothers you I would leave it and call it an original.

The inclusion of the clear coat, and application spikes makes it a premium kit I believe.

I walked on it tonight for the first time after the clear coat, and I hardly notice the fisheyes unless I look very closely. Here's a closeup pic of the fisheyes after the clear coat. No, it doesn't really bother me, and I'll just leave it alone. I have 55sqft left to do this weekend where the washer/dryer is currently at, and I'll make sure I backroll it more aggressively this time. If I were to do this again, I would definitely prime it first.

Anyway, thanks to everyone for their replies. I just found out about this forum a month ago, and have learned so much here. At least now I know what fisheyes are.

I think it look great, and the little gray spots are well hidden by the speckles. My only concern is how well it will hold up if there were so many places with issues?

I LOVE simple green for cleaning tiles BUT as Jorgen pointed out one time it leaves a residue. I wonder if that was the problem here.
