OK - here is the deal - I am putting a couple of fans on the peak of a cathedral ceiling - I have a bracket between the trusses to hold the box, and I have a 6" wide board going down the middle so the housing (canopy) of the fan will mount directly to it.
The problem I have (as shown in the picture) is that the electrical box does not come down flush with the mounting board - so
What am I supposed to do (for code) do I need a box extension and maybe a 2"X6" mounting block, or is there another simple solution. I do not think it would be good like it is.

Thanks

You can get a deeper box or replace that wood piece and screw a fan rated pancake box to it.

There is a "spark arrestor"/box insert thing that would work perfect for you there. It's basically a plastic extension that takes up that gap.

I've used a couple of these before and they are nice. Not many even in the trades know about these.

Has nothing to do with a fan box and wouldn't do anything in this situation. Electricians know about these trust me. We know all the parts related to the trade.

The fan box has to be flush with the finished ceiling for code and for the fan bracket to be safely secured.

I would shim behind that box with a piece of plywood of proper thickness. That'd be the easiest route. Holesaw yourself a 4" donut out of a plywood scrap and insert it up through the hole behind the box.

Is the box you used designed or rated to support a fan?

Yes - I got a fan kit with a metal brace that reaches the 22" between studs and had a metal box that mounted to it. It is pretty sturdy. The box is 1 1/2" deep - I think they make a 2 1/4" deep box that will mount to it and fit pretty well.
