Fastener with Washer

What exactly is this called and where can I get one? The size it not critical, something around 3/8" or 1/2".

That system is a ball screw and nut. Typically used on CNC applications, resist backlash.

You can find lots on Ebay.

Also look on Mcmaster.com, best place to do research on stuff you dont know about.
