If you measure the thickness of the lamp inches, it will tell you what size it is.
Every 1/8" = 1 so a 1-1/2" thic lamp is a T-12, a 1" = T-8, a 5/8" = T-5.

the tag is worn on the balast so I cannnot make it out., also the one balast fires all 4 bulbs. there is a small strait bulb and the three arched.
the bulbs are not readable either. all I can make out on one bulb is volt arc.
is there a way to test one of the bulbs?
or test the balast?

That lamp has a recessed double contact base, which means it is a HO, High Output lamp, that one is going to be a tough one..... (Uncommon type). $$$

Recessed double contact bases are used on HO 800ma, & VHO 1500ma lamps, I doubt the sign lamps are VHO.


Edit: Sign ballasts are rated in the number of lamps & the number of feet of the lamps, for example, 2,3 or 4 lamps and minimum 6 feet & maximum 20 feet of lamp length for a Advance brand model # ASB-0620-24-BL-TP ballast.
