Flush cleanout in floor

I'm getting ready to pour concrete and having trouble finding a cleanout to put through the floor of my garage. I will also need one in the driveway and another out in the yard. The only cleanouts I'm finding are for light foot traffic. I have 4" PVC drain run with risers.

(Also why no plumbing section?)
