Haha... I don't think its for clothes, this thing weighs at least 50-60 pounds. I am pretty sure when the top is turned the top cylinder turns. I will check tomorrow to make sure.

I think it is for thinning non ferrous metals
you can take a copper/silver/brass/etc sheet and thin the material for several purposes

A ring roller.

Why, you may ask, would you want to roll rings? Think of the metal "tire" for a wagon wheel.

Here is another variation:

I don't think it's a ring roller - there are only two wheels, and you need three to roll a ring. It really looks like it's for flattening or thinning something. The upper roll moves up or down when the crank on top is turned. The lower roller is missing a gear, and there was probably a handle at one time that turned the rolls.

The top picture is a bar thickness roller used for soft metals from brass to gold and such.

The second one Pa Toyota posted is a carrige tire roller. A wagon tire roller is a lot heavier construction, and generally bolted to the floor or a very large plank. The operating handle will be about 5 or 6 feet long, depending on the weight of the wheelright & his helper.

Cool thanks guys I would have never figured it out without the help. I think I might sell it on Ebay for him. We have no use for it.
