I have tried searching YouTube videos and looking for online resources, but I haven't found exactly what I am looking for yet. For a monolithic slab foundation, can anyone point me toward some online resources (videos/websites) that describe site preparation/requirements?

A more specific question, I am looking to do a 40x60 or so slab with footing and the frost line is 12 inches. Code states that the bottom of the footer must extend below the frost line, but it doesn't seem to define how far it must extend below. What is generally done?
