Frost free hydrant identification repair

I have a frost free hydrant that is quite old and installed by previous owner. Unfortunately they poured concrete around it and it is leaking.

Can anyone help me identify this and source parts?

I am hoping not to have to remove the concrete and would like to repair in place if possible

Ayuh,..... Where is it leaking,..??..??

With the water main, Off, loosen the bolt holdin' the down rod in the center, Unhook the hose, with 2 pipe wrenches, turn the faucet off the down pipe,.....

Then the down rod can be pulled up, 'n the packin' seal at the bottom can be replaced,....

Do a little clean up in the indented part of the nozel. The manufacture should be stamped in that location.. If cleanup doesn't work try a paper rub. Will go a long way to getting the help you are requesting.
