GE THQL2130GFT GFCI breaker installation /problems

Hi,
I suspect this breaker is defective but in my hart hope I'm wrong.
Pushing the Test button does not interrupt power. And here is red LED inside. White wire connected Ground/Neutral (it's main panel)
Both Red and Black both read 115V to Neutral.
Did I miss anything?

Just to confirm the white pigtail is going to the panel neutral bar? There isn't anything connected to the neutral load terminal?

I would try to disconnect the load and see if after you cycle the breaker if it will reset & trip properly.

Load removed, only pigtail connected to to the panel neutral bar. It was cycled. Nothing changed. So it defective, right?

A couple of years ago, I had a GE 120/208 3Ø 4wire bolt-on panel with about 15 2 pole GFI breakers factory installed. After it was energized, every breaker tripped.

Even after I disconnected all the load wires, still every breaker tripped.

If I disconnected the white wire from the neutral bus, they held. But would not trip on an actual ground fault.

Eventually, it was replaced by a Square D panel and all breakers did just what they were supposed to do.

This one never trips. installed in GE panel and breaker is listed as compatible. I have 15 A single pole GE GFCI installed, it works as expected.

Not new, was given to me without any history. I was hoping to avoid $100+ trip to store

I recently bought/threw out 2 THQL 20 amp GFCI breakers that were always tripping for no reason. No load on them at all. Bad out of the box.

I replaced them with GFCI receptacles at the head of the load circuits.
