Anyone know what it's called or where to get some?

A friend used to have one and it worked great for setting anchors like this one where you have to pound in the outside body before you set the center nail.

I've run across several instances where a hollow punch made for metal would have come in handy, but I've never been able to find anything. I almost need a small piece of thick-walled pipe

This is where you need a friend with a lathe and a piece of solid round. Harden, quench + temper both ends or case-harden with kasenit and profit with the rest of your day. Even made soft from something like W1 drill rod you'll get quite a few hits.

I have never had to really hammer them hard. As said, claw hammer works great. If you really need to hammer it hard just pull the pin first.
