I could be wrong, but as far as I know there is no code for that.
as mentioned never heard of a code, but that wasn't your question. I would say a foot or so

They can be up against the wall as long as they're two way or three way throw.

Depends on your grilles and ceiling construction.
Round ones need more room than rectangular ones. Mount for ease of installation.
Round could use a 6" elbow where rectangular needs a boot.

Leave them at least 6" out so you have an option to put up crown molding. Otherwose, there are no rules / codes
