Looks like it's been stuck in the vents of a brake disc to stop it turning while tightening/loosening a hub nut. Just one possibility.

Looks to me like the poor guy was pushing the driver correctly into the screw and the shaft must have just buckled.

I bought a tool box full of tools and anything in that had a long shaft that could be used to pry was bent, I mean everything, speeders, screwdrivers, extensions, etc, looks like you've got one those guys.

I acually have a few screwdrivers that look like that but they are cheap Stanley ones.
