OK, so first you need to download this image, courtesy of vintage machinery dot org:

Next, measure the switch plate. I used a standard plate for a "handy box" . I decided that 2" x 4" would work.

Import image into Word and scale the image to those dimensions.

You will need some "Sticker Project Paper": I used Avery number 4397. You can get this here:

Print the image using your ink jet printer on the best/finest image setting.

Let is dry: I gave it 24 hours.

To make sure the water-based ink would not smudge or otherwise degrade, I sprayed the decal/sticker with some Krylon acrylic spray paint: you can get this here:

Krylon Paint

One dry, cut the decal to the proper size, and apply it to the switch plate (I cleaned the plate with DNA prior to adhering the sticker).

Once its adhered, give it another coat of acrylic finish, to seal the sticker (particularly the edges) to the plate.

Install the plate and enjoy!
