Someone may make them, but most are probably custom/ home made. Typically made to be used by a black smith/ blade smith and are used to twist railroad spikes and other square stock. I've never seen one manufactured outside of DIY.

Interesting that it incorporates an adjustable wrench. Every time I’ve seen them in blacksmith’s shops as @SMOKEYBEAR mentioned they are custom made for the size of the stock.
When you're hammer forging damascus, the "size" depends on what step you're at. Having an adjustable wrench makes good sense, and having two handles to balance the force when twisting also makes good sense.

Looks like a second handle / bar was welded onto the head of an adjustable wrench...
I see some weld porosity on the head and some grinding / polishing on the add on piece...
Seems to work well for her purpose...

My buddies just weld a bar on their adjustable wrenches. They want me to give them old wrenches to make them.

Do I offer them? Diamond Calk, Proto, Williams..... F NO.
