I Picked this up used in a lot of tools and wonder how useful it is before I decide to keep it. It's the 3/8" version. I already use the dewalt 12v max system so that's a bonus. It seems a little weak on power and is a little bulky.

Is this worth keeping?

Its on the long side, about the only thing wrong with it. Head is nice and small so I wouldn't call it bulky at all. You don't buy a cordless ratchet for power, its for the convenience of being able to spin a fastener in and out at 200-300RPM. You can also use it like a regular ratchet to break a fastener loose.

I think cordless ratchets are definitely handy to have.

I've got a 1/4" milwaukee. It's my most used cordless tool...

I'd either keep it or sell it and buy a 1/4 inch

I can't speak for the Mac, but I really like my 3/8" cordless SO. Once you get a feel for its power, you will find the appropriate times to use it. If something is to tight for it to turn, I've had no trouble breaking stuff loose with. I guess try it for yourself and see if it fits your needs.
