After doing this for as many years as you have been in concrete you need to be weary of any floor in a basement, low lying area, side of a hill, etc...
In these cases test but I would use a Moisture Vapor Barrier Coating no matter what. It's cheap insurance. Testing is not always effective as water can be intermittent.
Our area is full of lakes and low lying areas. Ground water is only 20' below grade. Dig a hole and it fills with water. We use our MVB 20% of the time as you get a sense for when it is needed after a while.
