Multitool 2x36 issue

I've had the Multitool 2x36 attachment on my grinder for 1.5-2 years. It's always worked great until recently. The belt does not want to track straight, I have to move the adjustment lever as I turn the grinder on/off because the belt will move side to side when starting/stopping.

Thoughts on what's wrong? I can't find anything that's loose.

I have used these for 12 plus years and occasionally, I have to make adjustments. These are what I normally do when I have this issue:

Look at where the rotating plate mounts to the tool base. If I remember correctly (I'm not in the shop) there are three bolts that tighten that flat. Loosen those and turn the grinding attachment and make sure it is seated well. Torque evenly. That could be the cause. Also check for hub run out. You can loosen the hub and rotate it as well just to verify that it is seated correctly as well. The next thing that comes to mind is a worn front contact wheel. Any of things are possible. Other possible problems could be a defective belt or front contact wheel bearing wear. Let us know what you find.
