Need help on a tool

I picked this up recently as part of a box of loose tools at a yard sale. How would you use this adapter

with two female ends?

I think that went to an older set of double male end extensions. I'm pretty sure I have a set of older Williams socket set with that same adapter.

might well be. somebody just posted a photo of a 1/2" drive extension with two male ends just a couple days ago, asking what it was.
some older female drive ratchets used a double-male-end drive extension to connect to the socket.

are there detents at both ends of that thing?
if there are detents on one end only I'd be more inclined to say square socket.
if there are detents on both ends, I'd say ken's correct.

That's a female universal joint. As Ken said, it went with the double male extensions. I believe it is Indestro brand. There is one in the No. 1536 set at AA, bottom of the page:

Last week I picked up a Hinsdale Socket set which as an internal hex in lieu of 3/8 female. Regardless, while attempting some research, I came across the attached Ebay ad which has this in the socket kit.

Are you sure it isn't just an universal socket? One side looks hex to me?
